All league forms are now available to download from the League Website as editable PDF files which have been created for us by league sponsor Corsham Print. The forms can be accessed by clicking on the pictures of the forms shown below.


League Application Form


Purpose: For new teams wishing to join the league for the following season. 


Send to: League Secretary at the following email address: jim.thorn@wiltshireseniorleague.co.uk


Timing: All applications must be submitted by 30th April.

Postponement Form


Purpose: For confirming reason as to why a game has been postponed in order to comply with one of the requirements of Rule 20 (E) (iii).  Submission of this form is mandatory for all postponements for whatever reason.

Timing: Within 3 days of the postponed game.

Referee Registration Form


Purpose: For all referees wishing to officiate on the Corsham Print Wiltshire Senior League.


Send to: Referees' Secretary at the following email address: darren.oneil@wiltshireseniorleague.co.uk


Timing:  To be sent each season prior to being able to be appointed to games in any division of the league for that season.

League Agreement and Club Details Forms


Purpose: To provide details of club officers and comply with FA Standard Rule 10 and provide details for purposes of the League Handbook and set up of FA Full-Time for the coming season.

Timing:  To be sent each season prior to 1st July. Failure to submit will result in fines in accordance with the fines tariff.
